RN.
Key Responsibilities
Evaluate member needs and facilitate personalized care plans
Coordinate care and resources between members, families, and
providers
Monitor member progress and update care plans as necessary
Required Qualifications
Degree from an Accredited School of Nursing or a Bachelor's degree
in Nursing
2 - 4 years of related experience
RN - Registered Nurse - State Licensure and/or Compact State
Licensure required
