Job Description Job Description Position Summary The Case Coordinator is responsible for responding to referral sources and facilitating all activities related to the admission process in the Acute Rehab Unit. Required Qualifications One (1) year of related experience. Excellent written and verbal communication skills in English. Ability to communicate in a concise and effective manner. Ability to maintain a work pace appropriate to the workload. Ability to work independently with minimal supervision and direction. Must demonstrate customer service skills appropriate to the job. Demonstrates excellent organizational skills/abilities. Ability to establish and maintain effective working relationships. Valid California Driver’s License and reliable/insured vehicle. Preferred Qualifications Registered Nurse and/or Rehabilitation Certification or licensed therapist in other allied health area. Previous experience working in healthcare program development in Los Angeles or Orange County market. Physical Requirements These are requirements normally expected to perform regular job duties. Functions as liaison to the referral source. Establishes and maintains excellent rapport and working relationship with all referral sources. Provides all required follow-up activities promptly and accurately, such as: Advises patient and family of acceptance to rehab. Advises Nursing Unit of patient’s acceptance by documenting in Progress Notes. Adives Nursing Unit of patient’s denial and reason with a recommendation of a more appropriate course of action, IE: skilled unit, outpatient, etc. Responds to both internal and external patient referral request timely. Provides information in daily admission planning meeting. Provides case management services to insure the necessary and appropriate provision of services. Ensures HIPAA compliance at all times. Accurately obtains information regarding patients’ eligibility by: Chart review. Financial review. Patient interview to determine functional abilities, current and prior to onset of symptoms, and initial discharge plan. Explanation of rehab concept to patient and family, particularly amount of therapy to expect. Establishes effective communication with appropriate members of the medical community, discharge planners, case managers, local industry, and other consumers. Participates in community and professional activities. Recommends new services and programs to meet unfulfilled needs and improve service delivery and public relations. Develops and maintains patient review program to assess clients perception of the program and identify perceived problems. Develops and maintains system for communicating performance outcomes to stakeholders. Participates in design of all educational material for dissemination of information to patients and other consumers.
