About the Role: The Case Manager BHU LVN at Mission Community
Hospital plays a critical role in coordinating and managing patient
care within the Behavioral Health Unit. This position ensures that
patients receive comprehensive, individualized care plans that
address their physical, emotional, and social needs. The Case
Manager collaborates closely with multidisciplinary teams including
physicians, nurses, social workers, and external agencies to
facilitate seamless care transitions and optimize patient outcomes.
By monitoring patient progress and adjusting care plans as
necessary, the Case Manager helps reduce hospital readmissions and
supports long-term recovery. Ultimately, this role is essential in
promoting patient well-being and enhancing the overall quality of
behavioral health services provided by the hospital. Minimum
Qualifications: Valid Licensed Vocational Nurse (LVN) license in
the state of California. Minimum of 2 years of experience in
behavioral health nursing or case management. Basic knowledge of
behavioral health disorders and treatment modalities. Strong
understanding of patient confidentiality and HIPAA regulations.
Proficiency in electronic medical records (EMR) systems. Preferred
Qualifications: Certification in Case Management (CCM) or
equivalent. Experience working in an inpatient behavioral health
unit. Familiarity with community mental health resources and social
services. Advanced training in crisis intervention and
de-escalation techniques. Bilingual abilities, particularly in
Spanish, to better serve the local community. Responsibilities:
Conduct thorough assessments of patients’ behavioral health needs
and develop individualized care plans. Coordinate with healthcare
professionals and community resources to ensure continuity of care.
Monitor patient progress and update care plans accordingly to
reflect changes in condition or treatment goals. Facilitate
communication between patients, families, and care teams to support
patient-centered care. Document all case management activities
accurately and maintain compliance with hospital policies and
regulatory requirements. Assist patients in accessing necessary
services such as housing, counseling, and medication management.
Participate in multidisciplinary team meetings to discuss patient
care strategies and discharge planning. Provide education and
support to patients and families regarding behavioral health
conditions and treatment options. Skills: The Case Manager BHU LVN
utilizes clinical nursing skills daily to assess patient needs and
implement care plans effectively. Strong communication skills are
essential for collaborating with multidisciplinary teams and
engaging patients and their families in the care process.
Organizational skills enable the Case Manager to manage multiple
cases simultaneously while ensuring accurate documentation and
compliance with regulatory standards. Problem-solving abilities are
applied when coordinating resources and addressing barriers to
care. Additionally, cultural competence and empathy are critical in
providing respectful, patient-centered care that meets the diverse
needs of the hospital’s behavioral health population.
