Job Description:
A company is looking for a Transplant Coordinator Advanced Lung
Disease.
Key Responsibilities
Assess patient needs and collaborate with the healthcare team to
address care concerns
Document care plans and outcomes, ensuring compliance with policies
and regulatory guidelines
Educate patients, families, and care providers on care aspects and
maintain data accuracy for reporting
Required Qualifications
Bachelor's Degree in Nursing
3 years of experience as a Registered Nurse
Licensed as a Registered Nurse in the Commonwealth of Virginia
Basic Life Support certification from the American Heart
Association
On-call requirement every 10th weekend for remote patient
triage
